Information on the Independent Intermediary
Eurovalley s.r.o., Company ID: 293 68 324, with its registered office at Příkop 6, 602 00 Brno, is a registered independent intermediary with the CNB, in accordance with Act No. 170/2018 Coll., on insurance and reinsurance distribution (hereinafter referred to as "ZoDPZ"). In accordance with Act No. 170/2018 Coll., on insurance and reinsurance distribution (hereinafter referred to as "ZoDPZ"), the Intermediary holds authorization for the activity of an independent intermediary1,2, granted by the Czech National Bank, with its registered office at Na Příkopě 28, 115 03 Prague 1 (hereinafter also referred to as "CNB"). The CNB supervises the activities of the Intermediary and its tied agents (hereinafter referred to as "TA"1). The Intermediary and its TA are registered in the register maintained by the CNB. Customers can best verify the registration of the Intermediary and its TA in the CNB register by contacting the CNB directly via its website www.cnb.cz or by phone at +420 224 411 111.
The Intermediary will use the Czech language for communication with the customer. The customer will receive documents and other information from the Intermediary in this language. Customers can communicate with the Intermediary in person, by phone, in writing, or via email. Customers have the option to choose whether information stipulated by legal regulations will be provided by the Intermediary through paper records and messages or electronically via another durable medium. By signing this document, the customer confirms that both options for providing information were offered to them and that they agree to the provision of information via electronic communication means (another durable medium) and that the chosen method is suitable given the insurance being arranged. Some information will be provided to the customer in accordance with legal regulations via the Intermediary's website www.eurovalley.cz, to which the customer agrees by signing this document. In this regard, the customer declares that they have regular internet access, regularly use this service, and have the opportunity to familiarize themselves with the information provided on the Intermediary's website and through electronic communication means, and that they have an email address. If the customer does not have regular internet access, they are obliged to inform the Intermediary of this fact without delay. If the customer requests information to be provided in paper form, they will communicate their request to the Intermediary, and the Intermediary will provide the information in that form to the customer.
In accordance with ZoDPZ, the Intermediary issues rules for identifying and managing conflicts of interest (hereinafter referred to as the "Conflict of Interest Management Rules"). The purpose of these Rules is to effectively identify and manage conflicts of interest during insurance intermediation to prevent any detriment to customer interests. If a conflict of interest cannot be avoided, the Intermediary will always prioritize the customer's interests over its own interests or the interests of persons financially or otherwise connected with the Intermediary. A specific type of conflict of interest involves inducements. The Intermediary and its TA must not, during insurance intermediation, accept, offer, or provide any monetary or non-monetary benefit that could lead to a breach of obligations stipulated by ZoDPZ or other legal regulations, including unusual remuneration for insurance intermediation or any other unjustified financial, material, or non-material benefit. Neither the Intermediary nor its TA hold a direct or indirect share in the voting rights or share capital of insurance companies with which the customer may arrange insurance through the Intermediary. Furthermore, no insurance company with which the customer may arrange insurance through the Intermediary, or any person controlling such insurance company, holds a direct or indirect share in the voting rights or share capital of the Intermediary or its TA. More detailed information on conflicts of interest and inducements will be provided by the Intermediary upon customer request, and this information is also published on the Intermediary's website.
Information on Insurance Intermediation
The Intermediary mediates insurance for customers either as an insurance agent, based on a written agreement with insurance companies, or as an insurance broker, based on a written agreement with the customer. The Intermediary may mediate insurance for insurance companies listed in the CNB register as cooperating insurance companies of the Intermediary and on the Intermediary's website www.eurovalley.cz. The customer's requirements, objectives, and needs related to the arranged insurance, the recommended insurance product, the reasons on which the Intermediary bases its recommendations (to enable the customer to decide whether to arrange or substantially change the insurance), and any warnings about discrepancies between the customer's requirements and the offered insurance are all detailed in the document "Record of Customer Requirements, Needs, and Objectives". The Intermediary and its TA are remunerated in the form of commissions by cooperating insurance companies in connection with the arranged insurance or changes to insurance. Neither the Intermediary nor its TA are directly remunerated by the customer. When mediating insurance, the Intermediary does not provide advice to the customer within the meaning of Section 78 of ZoDPZ. When mediating insurance, the Intermediary provides recommendations within the meaning of Section 77 of ZoDPZ, with the exception of cases listed in Section 77, paragraph 3 of ZoDPZ.
The Intermediary provides the client with regular reports on its services, commensurate with the type of agreed insurance products, including costs associated with providing these services and with transactions carried out on behalf of the client. A complaint or grievance regarding the conduct of the Intermediary and its Tied Agent (VZ) may be submitted by the client, insured person, or other authorized individual in person, in writing, or by email to the Intermediary's contact details. These individuals also have the option to contact the Intermediary's supervisory authority, which is the Czech National Bank – www.cnb.cz, or to file a lawsuit against the Intermediary with the competent court, or to turn to an out-of-court dispute resolution body, which for life insurance is the Financial Arbitrator – www.finarbitr.cz, and for non-life insurance is the Czech Trade Inspection Authority – www.coi.cz. The detailed procedure for submitting and handling a complaint or grievance addressed to the Intermediary is governed by the complaints policy located on the Intermediary's website.
